The purpose of this agreement/disclaimer is to go over expectations, boundaries, and share

necessary information right from the get-go! Magic happens in these containers- and creating a

space where growth, joy, and possibility are held central is important to me. By participating in

Creative Mentoring with Eliza White, MA / Eliza Lois Creative you agree to the the following:

1. 2. 3. 4. 5. Service. Creative Mentoring is a comprehensive process that may involve different areas

of your life, including work, creative practice, health, relationships, and so on. You

understand that both the level of information you share and how you choose to proceed

with implementing our discussions is exclusively at your discretion. The most effective

mentoring relationships are centered around honest communication, openness to

feedback, and creating the time and space necessary to fully participate in our work

together. I am excited to fulfill the services outlined in this package!

Responsibility. Creative Mentoring is in no way to be construed or substituted as

psychological counseling, psychotherapy, mental health counseling, or any other type of

psychotherapy or medical advice. Eliza is not a therapist, and nothing we’re doing here is

intended to prevent, cure, or treat mental health or disorders or medical diseases- it is

your exclusive responsibility to seek independent professional guidance if and as needed.

Liability. Eliza (while being damn good at what they do) cannot guarantee the outcome

or results of this program. No one is more of an expert on you than you! All decisions

around your own emotional well-being, choices, decisions, actions, and results will come

from you. As such, Eliza is not liable for any actions/inaction/results/outcomes that arise

from your participation in this program.

Investment. The time, energy, and emotional space that I hold for you is intentional and

sacred. Please set your appointments in advance and make every effort to keep them.

Should you need to reschedule a call, be in touch as soon as possible. Life happens and

sometimes I will need to reschedule with you! Every effort will be made to keep original

appointments and to reschedule in a timely manner. All payments must be made on a

timely basis as outlined in the original payment agreement determined at checkout. You

are responsible for paying the complete package fee, regardless of whether you continue

or discontinue the mentoring relationship for any reason.

Confidentiality. All information that you share with me is considered private and

confidential. However, this is not considered a legally confidential relationship (like a

doctor or lawyer) and thus communications are not subject to the protection of any

legally recognized privilege. I can’t imagine this will come up, but it’s important to me

that you know! I regularly share about my clients’ breakthroughs in my marketing, but

won’t use your name or tag you directly without prior consent.
